Interacts with interdisciplinary discharge planning team, medical, and nursing staff. Maintains performance improvement within the department and participates in C. Q. I. Provides crucial psychosocial support, helping patients and families cope with illness, navigate the complex healthcare system, and plan for discharge by assessing needs, offering counseling, arranging resources (like home care, financial aid), coordinating care with medical teams, and advocating for patient rights, ensuring holistic well-being alongside medical treatment. Licensure, Certification, Registration, etc.: Licensed Social Worker (M.S.W.) ### Special Skills, Knowledge Ability to communicate effectively in English and Spanish preferred. Strong written and verbal skills. Basic computer knowledge. Provide care appropriate to the age of the patient served, pediatrics, adolescents, adults, and geriatric patients.
