## Job description

### Responsibilities

Intake Assessment Coordinator (Behavioral Health) - Part Time- Neenah, WI We are looking for someone PT, 24 hours a week plus weekends Saturday or Sunday (weekends 10AM-6PM).

### Shift during the week

from 5PM-10PMish, 4PM-10PM. Plus Weekends either Saturday or Sunday (weekend both from 10AM-6PM). Horizon Health is seeking an Intake Assessment Coordinator (Part Time) to work in the Behavioral Health Program at Thedacare in Neenah, WI. The inpatient psychiatric unit has a total to 19 adult beds and intake team. The Intake Assessment Coordinator, under clinical supervision of a licensed social worker or licensed professional counselor, coordinates the referral development and pre-admission patient assessment process.

### Responsibilities

Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW. Facilitates groups, family and individual sessions and completing all necessary documentation. Schedules Unit assessments, meetings and appointments as appropriate. Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, LUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW.

Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities. Reviews newly admitted patient accounts for eligibility and authorization daily. Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships. Participates as an active member of the interdisciplinary team. Addresses issues in a timely manner including crisis and higher level of care referrals.

Performs other duties as assigned/required by this position.

- Bachelor’s Degree required 3-5 years of related work experience
- Proficient in software applications
- Ability to read and comprehend moderate to complex instructions and correspondence
- Ability to effectively present information in one-on-one and small group situations to patients and their families, facility leadership, employees’ clients, providers, vendors, etc.
- Ability to write detailed correspondence
- Excellent verbal and written communications skills
