Follows up on all referrals made to social work from all sources other than high risk screening within 24 hours of referral. Assesses patients to determine the bio-psychosocial factors that impact their medical condition, treatment, recovery, and safe transition from one care environment to another Develops intervention plan and discharge plan to meet patient’s/family’s needs. Provides social work services to patients in OB and takes weekend on call for OB/emergency department. Other duties as assigned. ### Qualifications Required: Education: Master’s degree in Social Work Experience: One year experience in an acute inpatient hospital setting or equivalent experience Licensure/Certification: LCSW eligible. ### Preferred Licensure/Certification: LICSW preferred Organization Description Saint Vincent Hospital has been providing high-quality healthcare to Worcester and the surrounding communities for more than 125 years.
