Follow the care plan after assessing the patient’s needs. Keep clear and timely records of the care provided. Teach and support patients and their families about the illness and care plan. Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers). Manage pain and symptoms, checking how well treatments are working. Take part in training and quality improvement activities to grow professionally and improve care. ### Responsibilities Benefits and Perks for You! Three years nursing experience. Community health/hospice or medical/surgical experience is preferred. Currently licensed as an LPN/LVN in the State. Comply with accepted professional standards and practice. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Possess and maintain current CPR certification in states where required. Understand philosophy of hospice concept. Also understand needs of the terminally ill. Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems. Demonstrate good verbal and written communication, and organization skills.
