APREVA HOSPICE is seeking a compassionate and creative Hospice Music Therapist in San Diego, CA. Our Music Therapists are a key part of our interdisciplinary team, providing comfort, connection and dignity to patients through the healing power of music. How YOU will benefit Flexible and inspired work—bringing your talents to life through therapy - Create a sense of peace for patients and families during final moments - Build connections and honor life stories - Ability to work independently while also having team support Job stability and regular advancement opportunities with a growing company As a Music Therapist You will: Assess patients’ functional status, music al responses, and preferences - Develop individualized treatment plans to help patients meet goals - Deliver music therapy using instruments, singing, movement, and listening - Monitor and evaluate patient progress regularly and update care team Maintain accurate clinical records and communicate changes in patient condition - Collaborate with interdisciplinary team members to ensure coordinated care - Participate in meetings, training, and quality improvement initiatives - Educate patients, families, caregivers, and staff on treatment plans Responsibilities Benefits and Perks for You!
